Northeast Georgia Physicians Group Orthopedic and Sports Medicine team will be offering a sports physical exam clinic on campus in the infirmary on April 30th. This clinic is available to rising 9th-12th grade student-athletes at no charge.
By attending this clinic, your son will meet the sports physical requirement for the upcoming 2025-2026 academic year. Please note that 9th grade students who will be attending the 9th grade trip on this date will be scheduled for their sports physical at a later date.
In order to take advantage of this opportunity, complete the first two pages of the attached form 2025 GHSA PPE Fillable.pdf. Aparent/guardian must complete and submit the completed form by email to infirmary@riversideprep.org or faxto 678-291-3365. The deadline for submission is April 25.
